I acknowledge that in accessing AlayaCare, I will have access to personal health information relating to clients of the Niagara Children's Centre. I acknowledge that I have been granted permission to use AlayaCare for the purposes of my official duties as an agent of the Niagara Children's Centre. I am responsible for maintaining the confidentiality of my login information. I agree to use AlayaCare in accordance with the policies and procedures of the Niagara Children's Centre. I will only access client information as necessary to perform my job function. I will only access AlayaCare from a Niagara Children's Centre encrypted computer. I will abide by applicable privacy laws, such as the Personal Health Information Protection Act, 2004 (PHIPA). I understand that access to AlayaCare is captured electronically and subject to random, as well as targeted audits.
